Alluvion Health has announced that its Project in Assistance in Transition from Homelessness (PATH) program is being revitalized with a new team of professionals. The program, which aims to support individuals experiencing homelessness, particularly those with mental illnesses or substance use disorders, will continue to offer outreach, engagement, case management, and service coordination services. The goal is to identify eligible individuals, assess their needs and connect them with resources to address their homelessness.
